Built for partner admins, care coordinators, and care navigation teams, Tava Guide helps teams move members from referral to first session with intuitive workflows, clear progress tracking, and better referral management all in one simple tool.
The updates were shaped by direct care coordinator feedback and are designed to give teams more flexibility and transparency in the moments that matter most: finding a provider, sharing options with a member, tracking progress after referral, and understanding team performance.
With the latest version of Tava Guide, care navigation teams can:
- Browse providers with 200+ filter options
- Schedule new sessions through two, flexible workflows
- Share provider options with a member by email
- Track performance with a Referral Management Dashboard
- Monitor recent movement through a live activity log
- Track member progress from first touchpoint to first session
- Review historical and future appointment details directly on the member profile
For many care coordinators, the work of connecting members to care happens in real time — often while talking with a member live, immediately after a call, or during fast-moving follow-up. Tava Guide now supports that reality more directly by allowing teams to follow intuitive workflows based on what the conversation requires.

On the individual member profile, Tava Guide now gives care coordinators a clearer picture of what has happened so far. Teams can use the member progress tracker to see movement from first touch through first attended appointment, and review appointment history and upcoming appointments for added context.
Over the next few months, additional Tava Guide capabilities are launching, including provider network importing and custom report exports, giving partner teams even more flexibility in how they manage networks and reporting workflows.
